Your initial appointment will consist of an interview with the doctor to discuss your concerns followed by an examination, cleaning of your teeth and an explanation of findings and recommendations for any necessary treatment as well as options and costs. An extensive medical history or complex dental issues may require further evaluation and a second appointment to thoroughly review conditions and treatment options.
Please assist us by providing the following information at the time of your consultation:
- X-rays if less than one year old
- A list of medications you are presently taking
- If you have dental insurance, please bring your insurance card with you. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during the initial office visit.
Please alert the office if you have a medical condition that may be of concern i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc. or if you are on any medication i.e. heart medications, aspirin, anticoagulant therapy, etc.
